Documents and Forms

There are 6 essential forms we need from you by May 1 to help us prepare for your camper’s arrival.

Here’s the breakdown:

• Online Forms (3 total) These are completely digital—fill them out anytime, anywhere (yes, even from the comfort of your couch in pajamas).

• Health Forms (3 total)  These need to be printed out. Form 1 can be filled out by a parent/guardian; Form 2 must be filled out by a medical professional; Form 3 is optional.
